I hereby, for myself, my heirs, my executors and administrators, waive and release all rights and claims for damages I may have against DC Ranch, its affiliates, employees, agents, directors, shareholders, partners, event sponsors, and any individuals associated with the event, their representatives, successors and assignees, and will hold harmless for any and all injuries or illness suffered in connection with the Run for Ryan House event. I attest that I am in good physical condition and capable of participating in this event. Further, I hereby grant full permission to any and all forgoing to use my likeness in all media including photographs, recordings, my story, and any other record of this event for any legitimate purpose. By signing, I acknowledge I have read and fully understand my own liability and do accept the restrictions. Incomplete or unsigned forms will not be accepted. If entrant is under 18 years old, one parent of legal guardian must sign.
